Social Support of Patients with Type 2 Diabetes in Marginalized Contexts in Mexico and Its Relation to Compliance with Treatment: A Sociocultural Approach

Objective This study aimed to describe the ways social support works in the daily life of patients with type 2 diabetes living in conditions of social and economic marginality, in order to understand how that support relates to treatment compliance. Methods Sequential mixed methods research was used. The sample of patients was obtained from primary health care units and selected considering regional representativeness, and levels of morbidity and mortality for type 2 diabetes. Results Results point to the nuclear family as the main source of support. Regardless of the area of residence, four main dimensions of support were identified: economic support, help with treatment compliance, emotional support, and material aid. Conclusions We conclude that the support network assists the patient in different ways and helps cope with the disease, but in conditions of social and economic marginality, does not guarantee the quality of attention nor enable the self-management of treatment.

Objectif Cette étude visait à décrire le fonctionnement du soutien social dans la vie quotidienne des patients atteints de diabète de type 2 vivant dans des conditions de marginalité sociale et économique, afin de comprendre comment ce soutien est lié à l'observance du traitement. Méthodes La recherche sur les méthodes mixtes séquentielles a été utilisée. L'échantillon de patients a été obtenu auprès des unités de soins de santé primaires et sélectionné en tenant compte de la représentativité régionale et des niveaux de morbidité et de mortalité pour le diabète de type 2. Résultats Les résultats indiquent que la famille nucléaire est la principale source de soutien. Quel que soit le lieu de résidence, quatre dimensions principales du soutien ont été identifiées : le soutien économique, l'aide à l'observance du traitement, le soutien émotionnel et l'aide matérielle. Objetivo Este estudio tuvo como objetivo describir las formas en que funciona el apoyo social en la vida diaria de los pacientes con diabetes tipo 2 que viven en condiciones de marginalidad social y económica, con el fin de comprender cómo se relaciona ese apoyo con el cumplimiento del tratamiento. Métodos Se utilizó la investigación de métodos mixtos secuenciales. La muestra de pacientes se obtuvo de las unidades de atención primaria de salud y se seleccionó teniendo en cuenta la representatividad regional y los niveles de morbilidad y mortalidad para la diabetes tipo 2. Resultados Los resultados apuntan a la familia nuclear como la principal fuente de apoyo. Independientemente del área de residencia, se identificaron cuatro dimensiones principales de apoyo: apoyo económico, ayuda con el cumplimiento del tratamiento, apoyo emocional y ayuda material. Conclusiones Concluimos que la red de apoyo asiste al paciente de diferentes maneras y ayuda a sobrellevar la enfermedad, pero en condiciones de marginalidad social y económica, no garantiza la calidad de la atención ni permite la autogestión del tratamiento.Files
