Description
Antecedentes: El pene palmeado es una anormalidad congénita o adquirida, donde una cuerda escrotal se extiende a la base ventral del pene, en la población pediátrica suele no dar sintomatología severa, pero causa grandes molestias en la vida adulta, su correcta clasificación es fundamental para planear su corrección al momento de su detección. Objetivo: Este trabajo pretende describir una nueva clasificación para pene palmeado, modificando la ya existente, cambiando completamente la perspectiva de esta patología y agregando una variante de pliegues anormales a la clasificación mas aceptada. Se pretende que esta nueva clasificación ayude a la elección de técnicas quirúrgicas específicas. Pacientes y métodos: Se realizo un análisis de la clasificación de El-Koutby y El Gohary para pene palmeado y se modificó con base en su descripción anatómica y evidencias de la patología en diferentes bibliografías, se añadió una variante a la clasificación por un caso que se presentó en nuestro hospital. Resultados: Se elaboro una nueva clasificación que abarca anormalidades que se presentan con el pene palmeado y que son importantes para la planeación quirúrgica. Además, se cambió el concepto de pene palmeado por cuerdas peneanas y se incluyó la patología reportada que no estaba contemplada. Conclusión: El cambio del término pene palmeado por el de cuerda peneoescrotal y pubopeneana, comprendido dentro de las cuerdas peneanas, se consideró más adecuado por la ambigüedad y múltiples sinónimos de esta patología, además la modificación efectuada a la clasificación permite ubicar a los pacientes para una selección de técnica quirúrgica.

Background: Webbed penis is a congenital or acquired abnormality, where a scrotal cord extends to the ventral base of the penis, in the pediatric population it usually does not give severe symptoms, but causes great discomfort in adult life, its correct classification is essential to plan its correction at the time of its detection. Objective: This work aims to describe a new classification for webbed penis, modifying the existing one, completely changing the perspective of this pathology and adding a variant of abnormal folds to the most accepted classification. This new classification is intended to aid in the choice of specific surgical techniques. Patients and methods: An analysis of the El-Koutby and El Gohary classification for webbed penis was carried out and modified based on its anatomical description and evidence of the pathology in different bibliographies, a variant was added to the classification for a case that occurred in our hospital. Results: A new classification was developed that covers abnormalities that occur with the webbed penis and that are important for surgical planning. In addition, the concept of a webbed penis was changed to penile cords and the reported pathology that was not contemplated was included. Conclusion: The change of the term webbed penis to penile scrotal and pubopenean cord, included within the penile cords, was considered more appropriate due to the ambiguity and multiple synonyms of this pathology, in addition to the modification made to the classification allows patients to be located for a selection of surgical technique.Translated Description (French)

Objectif : Ce travail vise à décrire une nouvelle classification pour le pénis palmé, en modifiant celle déjà existante, en changeant complètement la perspective de cette pathologie et en ajoutant une variante de plis anormaux à la classification la plus acceptée. Cette nouvelle classification est destinée à aider au choix de techniques chirurgicales spécifiques. Patients et méthodes : Une analyse de la classification d'El-Koutby et d'El Gohary pour le pénis palmé a été réalisée et modifiée en fonction de sa description anatomique et des preuves de la pathologie dans différentes bibliographies, une variante a été ajoutée à la classification pour un cas présenté dans notre hôpital. Résultats : Une nouvelle classification a été élaborée qui couvre les anomalies qui se produisent avec le pénis palmé et qui sont importantes pour la planification chirurgicale.
