Interventions to Improve Clinical Coordination between Levels: Participatory Experience in a Public Healthcare Network in Xalapa, Mexico

Coordination of care can be improved through an intervention or a combination of several ones. In addition, it is recommended to encourage the active involvement of professionals in the design, implementation and assessment of coordination mechanisms.To analyse the factors that influence the implementation of participatively designed interventions and their effects on clinical coordination between levels of care in a public healthcare network of health services in Xalapa, Veracruz, Mexico.A qualitative, descriptive-interpretative study, for which individual interviews and discussion groups with a criterion sample of participants: Local Steering Committee and the Professional Platform. A content analysis, with mixed category generation and segmentation by intervention and topics, was carried out. According to the problem analysis, participants designed two sequential interventions: offline virtual consultation, and joint training meetings on maternal health and chronic diseases.Respondents perceived a differentiated impact on clinical coordination according to intervention: greater in the case of joint maternal health trainings and limited for the chronic diseases meetings, as they were the offline virtual consultation was rarely used.The involvement of professionals in designing the interventions, as well as institutional support and reflexive methods for training, all decisively improved clinical coordination between levels.

La coordination des soins peut être améliorée par une intervention ou une combinaison de plusieurs interventions. Analyser les facteurs qui influencent la mise en œuvre d'interventions conçues de manière participative et leurs effets sur la coordination clinique entre les niveaux de soins dans un réseau public de services de santé à Xalapa, Veracruz, Mexique. Une étude qualitative, descriptive-interprétative, pour laquelle des entretiens individuels et des groupes de discussion avec un échantillon de critères de participants : le comité de pilotage local et la plateforme professionnelle. Une analyse de contenu, avec génération de catégories mixtes et segmentation par intervention et sujets, a été réalisée. Selon l'analyse du problème, les participants ont conçu deux interventions séquentielles : la consultation virtuelle hors ligne et les réunions de formation conjointes sur la santé maternelle et les maladies chroniques. Un estudio cualitativo, descriptivo-interpretativo, para el cual se realizan entrevistas individuales y grupos de discusión con una muestra criterio de participantes: Comité Directivo Local y la Plataforma Profesional. Se realizó un análisis de contenido, con generación de categorías mixtas y segmentación por intervención y temas. De acuerdo con el análisis del problema, los participantes diseñaron dos intervenciones secuenciales: consulta virtual fuera de línea y reuniones de capacitación conjuntas sobre salud materna y enfermedades crónicas. Los encuestados percibieron un impacto diferenciado en la coordinación clínica de acuerdo con la intervención: mayor en el caso de las capacitaciones conjuntas de salud materna y limitado para las reuniones de enfermedades crónicas, ya que rara vez se utilizó la consulta virtual fuera de línea.
