Effectiveness of secondary furlow palatoplasty with buccal myomucosal flap in correction of velopharyngeal insufficiency in patients with cleft palate

Abstract Objectives The main purpose of this study was evaluation of the effectiveness of secondary furlow palatoplasty with buccal myomucosal flap (FPBF) for the treatment of velopharyngeal insufficiency (VPI) in patients with a cleft palate who were treated with two flap palatoplasty (TFP) in their primary palate repair. Material and methods Twenty-three medically free children aged 4–8 years with non-syndromic and previously repaired cleft palate via TFP participated in the study. All patients received secondary surgery following the technique of FPBF. Preoperative speech evaluation was done before the secondary repair and 3 months after the surgery using a hypernasal speech scale, speech intelligibility scale, and nasopharyngoscopy. Results A statistically significant improvement was observed regarding the degree of hypernasality and speech intelligibility while comparing the preoperative scores after the primary surgery to the postoperative scores after the secondary surgery. In addition, a statistically significant improvement was found in the nasopharyngoscopic assessment. Conclusions The incorporation of a buccal myomucosal flap with Furlow palatoplasty was successful in improving hypernasality, speech intelligibility, and nasopharyngoscopic scores in patients with cleft palate. Trial registration clinicaltrials.gov (NCT05626933). Clinical relevance This technique might be the surgical technique of choice while treating patients who are suffering from VPI after cleft palate repair.

Résumé Objectifs L'objectif principal de cette étude était d'évaluer l'efficacité de la palatoplastie furlow secondaire avec lambeau myomucosal buccal (FPBF) pour le traitement de l'insuffisance vélopharyngée (IPV) chez les patients atteints d'une fente palatine qui ont été traités par palatoplastie à deux lambeaux (TFP) dans leur réparation palatine primaire. Matériel ET méthodes Vingt-trois enfants médicalement libres âgés de 4 à 8 ans présentant une fente palatine non syndromique et préalablement réparée via la PTF ont participé à l'étude. Tous les patients ont bénéficié d'une chirurgie secondaire suivant la technique du FPBF. L'évaluation préopératoire de la parole a été effectuée avant la réparation secondaire et 3 mois après la chirurgie à l'aide d'une échelle de parole hypernasale, d'une échelle d'intelligibilité de la parole et d'une rhinopharyngoscopie. Résultats Une amélioration statistiquement significative a été observée concernant le degré d'hypernasalité et d'intelligibilité de la parole en comparant les scores préopératoires après la chirurgie primaire aux scores postopératoires après la chirurgie secondaire. De plus, une amélioration statistiquement significative a été constatée dans l'évaluation rhinopharyngoscopique. Resumen Objetivos El objetivo principal de este estudio fue la evaluación de la efectividad de la palatoplastia de surco secundario con colgajo miomucoso bucal (FPBF) para el tratamiento de la insuficiencia velofaríngea (VPI) en pacientes con paladar hendido que fueron tratados con palatoplastia de dos colgajos (TFP) en su reparación primaria del paladar. Material y métodos Veintitrés niños médicamente libres de 4 a 8 años con paladar hendido no sindrómico y previamente reparado a través de TFP participaron en el estudio. Todos los pacientes recibieron cirugía secundaria siguiendo la técnica de FPBF. La evaluación preoperatoria del habla se realizó antes de la reparación secundaria y 3 meses después de la cirugía utilizando una escala de habla hipernasal, una escala de inteligibilidad del habla y una nasofaringoscopia. Resultados Se observó una mejora estadísticamente significativa con respecto al grado de hipernasalidad e inteligibilidad del habla al comparar las puntuaciones preoperatorias después de la cirugía primaria con las puntuaciones postoperatorias después de la cirugía secundaria.
