Clear cell adenocarcinoma of the male urethra: a case report

Abstract Background Primary urethral cancer in males is rare. Clear cell adenocarcinoma is more rare. We report a case in an African male suspected to have a urethral stricture. Case presentation A 66-year-old man presented in with preceding intermittent haematuria and acute urinary retention. Failed attempts at catheterisation necessitating a suprapubic catheter insertion raised the suspicion of a urethral stricture. Multiple irregular urethral filling defects were seen on a retrograde urethrogram. Urethroscopy revealed obstructing urethral masses. Histology reported clear cell adenocarcinoma. Conclusion Primary urethral cancer should be entertained as a differential diagnosis of a urethral stricture in a patient with haematuria, difficult urethral catheterisation and ambiguous urethrogram findings. Cystoscopy and biopsy are essential in the investigative work-up to make the distinction.

Nous rapportons un cas chez un homme africain soupçonné d'avoir une sténose urétrale. Présentation de cas Un homme de 66 ans a présenté une hématurie intermittente précédente et une rétention urinaire aiguë. Les tentatives infructueuses de cathétérisme nécessitant une insertion de cathéter suprapubien ont soulevé la suspicion d'une sténose urétrale. Plusieurs défauts de remplissage urétral irréguliers ont été observés sur un urétrogramme rétrograde. L'uréthroscopie a révélé une obstruction des masses urétrales. L'histologie a rapporté un adénocarcinome à cellules claires. Presentación del caso Un hombre de 66 años se presentó con hematuria intermitente previa y retención urinaria aguda. Los intentos fallidos de cateterismo que requirieron la inserción de un catéter suprapúbico levantaron la sospecha de una estenosis uretral. Se observaron múltiples defectos de llenado uretral irregular en un uretrograma retrógrado. La uretroscopia reveló masas uretrales obstructivas. Histología informada de adenocarcinoma de células claras. Conclusión El cáncer de uretra primario debe considerarse como un diagnóstico diferencial de una estenosis uretral en un paciente con hematuria, cateterismo uretral difícil y hallazgos ambiguos en el uretrograma.
