Hepatitis E Virus Genotype 3 in Colombia: Survey in Patients with Clinical Diagnosis of Viral Hepatitis

Hepatitis E virus is a major cause of outbreaks as well as sporadic hepatitis cases worldwide. The epidemiology of this enterically transmitted infection differs between developing and developed countries. The aims of this study were to describe HEV infection in Colombian patients and to characterize the genotype.A prospective study was carried out on 40 patients aged over 15 with a clinical diagnosis of viral hepatitis, recruited from five primary health units in the city of Medellin, Colombia. Fecal samples obtained from the 40 consecutives cases were analyzed for HEV RNA using nested reverse transcription PCR for both ORF1 and ORF2-3. The amplicons were sequenced for phylogenetic analyses.Nine (22.5%) cases of HEV infection were identified in the study population. Three HEV strains obtained from patients were classified as genotype 3. No significant association was found between cases of Hepatitis E and the variables water drinking source, garbage collection system and contact with pigs.This is the first prospective study of hepatitis E in Colombian patients. The circulation of the genotype 3 in this population is predictable considering the reports of the region and the identification of this genotype from pigs in the state of Antioquia, of which Medellin is the capital. Further studies are necessary to establish whether zoonotic transmission of HEV is important in Colombia.

Le virus de l'hépatite E est une cause majeure d'épidémies ainsi que de cas sporadiques d'hépatite dans le monde entier. L'épidémiologie de cette infection entérique diffère entre les pays en développement et les pays développés. Les objectifs de cette étude étaient de décrire l'infection par le VHE chez les patients colombiens et de caractériser le génotype. Une étude prospective a été réalisée sur 40 patients âgés de plus de 15 ans avec un diagnostic clinique d'hépatite virale, recrutés dans cinq unités de santé primaire de la ville de Medellin, en Colombie. Les échantillons fécaux obtenus à partir des 40 cas consécutifs ont été analysés pour l'ARN du VHE en utilisant la PCR de transcription inverse imbriquée pour ORF1 et ORF2-3. Les amplicons ont été séquencés pour des analyses phylogénétiques. Neuf cas (22,5 %) d'infection par le VHE ont été identifiés dans la population de l'étude. Trois souches de VHE obtenues auprès de patients ont été classées comme génotype 3. Aucune association significative n'a été trouvée entre les cas d'hépatite E et les variables source d'eau potable, système de collecte des ordures et contact avec les porcs. Il s'agit de la première étude prospective de l'hépatite E chez les patients colombiens. La epidemiología de esta infección de transmisión entérica difiere entre países en desarrollo y desarrollados. Los objetivos de este estudio fueron describir la infección por VHE en pacientes colombianos y caracterizar el genotipo. Se realizó un estudio prospectivo en 40 pacientes mayores de 15 años con diagnóstico clínico de hepatitis viral, reclutados en cinco unidades de salud primaria en la ciudad de Medellín, Colombia. Las muestras fecales obtenidas de los 40 casos consecutivos se analizaron para determinar el ARN del VHE mediante PCR de transcripción inversa anidada tanto para ORF1 como para ORF2-3. Los amplicones se secuenciaron para análisis filogenéticos. Se identificaron nueve (22,5%) casos de infección por VHE en la población de estudio. Tres cepas de VHE obtenidas de pacientes se clasificaron como genotipo 3. No se encontró asociación significativa entre los casos de Hepatitis E y las variables fuente de agua potable, sistema de recolección de basura y contacto con cerdos. Este es el primer estudio prospectivo de hepatitis E en pacientes colombianos. La circulación del genotipo 3 en esta población es predecible considerando los reportes de la región y la identificación de este genotipo a partir de cerdos en el estado de Antioquia, del cual Medellín es la capital.
