Poorly differentiated papillary thyroid carcinoma in a 5-year-old girl: a case report

Abstract Background The incidence of thyroid cancer has been increasing among the population less than 20 years of age, especially during the second decade of life. Differentiated papillary thyroid cancer is the most common pathology in children. It usually presents with thyroid nodules with or without cervical lymphadenopathy. Thyroid nodules carry a considerable risk of malignancy in children. Case presentation We herein report a 5-year-old girl with poorly differentiated papillary thyroid cancer which is an extremely rare finding at this age. The patient underwent right thyroid lobectomy followed by left hemi thyroidectomy and right central and lateral neck dissection. Pathologic examination revealed a poorly differentiated papillary thyroid carcinoma with nodal deposits. Conclusion It is essential to raise awareness of proper evaluation and management of thyroid nodules at such an age.

Résumé Contexte L'incidence du cancer de la thyroïde a augmenté parmi la population de moins de 20 ans, en particulier au cours de la deuxième décennie de vie. Le cancer papillaire différencié de la thyroïde est la pathologie la plus fréquente chez les enfants. Il présente généralement des nodules thyroïdiens avec ou sans adénopathie cervicale. Les nodules thyroïdiens présentent un risque considérable de tumeur maligne chez les enfants. Présentation de cas Nous rapportons ici une fillette de 5 ans avec un cancer de la thyroïde papillaire mal différencié, ce qui est une découverte extrêmement rare à cet âge. La patiente a subi une lobectomie de la thyroïde droite suivie d'une hémi thyroïdectomie gauche et d'une dissection centrale et latérale droite du cou. L'examen pathologique a révélé un carcinome papillaire de la thyroïde peu différencié avec des dépôts ganglionnaires. Los nódulos tiroideos conllevan un riesgo considerable de malignidad en los niños. Presentación del caso En este documento informamos de una niña de 5 años con cáncer de tiroides papilar poco diferenciado, que es un hallazgo extremadamente raro a esta edad. El paciente se sometió a lobectomía tiroidea derecha seguida de tiroidectomía del hemo izquierdo y disección del cuello central y lateral derecha. El examen patológico reveló un carcinoma papilar de tiroides poco diferenciado con depósitos ganglionares. Conclusión Es esencial crear conciencia sobre la evaluación y el manejo adecuados de los nódulos tiroideos a esa edad.
