Abstract Background The children's agency and that exercised by parents and health professionals in palliative care, along with structural limitations imposed by the conditions of inequality, will provide a new perspective from medical anthropology and biomedicine to improve pediatric palliative care in complex therapeutic scenarios.The main purpose of the study was to analyze the ways in which children have agency in relation to their parents and palliative care (PC) professionals within the hospital setting, as well as the structural circumstances that constrain said agency. Method A hospital ethnography (by means of non-participant observation and interviews) of the palliative care (PC) unit in a children's hospital was conducted over the course of six months. A thematic analysis was performed using the ATLAS.ti software . Results Thirteen cases were reconstructed of underage patients of both sexes boy and girl patients together with their families; five health professionals were interviewed. The analysis identified the following 6 thematic axes, around which this article is organized: 1. The relationship between the exercise of proxy agency and the medical decisions concerning underage patients. 2. Negotiating agency and support in decision-making. 3. Child autonomy. 4. The experiences of health professionals. 5. Limitations of palliative care. 6. Bureaucratization of palliative care. Conclusions In pediatric palliative care, agency is a process whereby different agencies intertwine: lack pediatric patients ´agency, the parents' agency, the parents' agency as representatives of their children (proxy agency), and the agency of health professionals. The concept of relational agency is proposed, defined as a set of group actions and decision-making centered around the children's agency and the proxy agency.

Résumé Contexte L'agence pour l'enfance et celle exercée par les parents et les professionnels de la santé en soins palliatifs, ainsi que les limitations structurelles imposées par les conditions d'inégalité, fourniront une nouvelle perspective de l'anthropologie médicale et de la biomédecine pour améliorer les soins palliatifs pédiatriques dans des scénarios thérapeutiques complexes. L'objectif principal de l'étude était d'analyser les façons dont les enfants ont une agence par rapport à leurs parents et aux professionnels des soins palliatifs (PC) en milieu hospitalier, ainsi que les circonstances structurelles qui contraignent ladite agence. Méthode Une ethnographie hospitalière (au moyen d'observations et d'entretiens avec des non-participants) de l'unité de soins palliatifs (PC) dans un hôpital pour enfants a été réalisée sur une période de six mois. Une analyse thématique a été réalisée à l'aide du logiciel ATLAS.ti. Résultats Treize cas ont été reconstruits de patients mineurs des deux sexes, garçons et filles, avec leurs familles ; cinq professionnels de la santé ont été interrogés. L'analyse a identifié les 6 axes thématiques suivants, autour desquels s'articule cet article : 1. Antecedentes abstractos La agencia infantil y la ejercida por padres y profesionales de la salud en cuidados paliativos, junto con las limitaciones estructurales impuestas por las condiciones de desigualdad, proporcionarán una nueva perspectiva desde la antropología médica y la biomedicina para mejorar los cuidados paliativos pediátricos en escenarios terapéuticos complejos. El objetivo principal del estudio fue analizar las formas en que los niños tienen agencia en relación con sus padres y profesionales de cuidados paliativos (CP) dentro del entorno hospitalario, así como las circunstancias estructurales que limitan dicha agencia. Método Se realizó una etnografía hospitalaria (mediante la observación y entrevistas a no participantes) de la unidad de cuidados paliativos (CP) en un hospital infantil a lo largo de seis meses. Se realizó un análisis temático utilizando el software ATLAS.ti. Resultados Trece casos fueron reconstruidos de pacientes menores de edad de ambos sexos, niños y niñas, junto con sus familias; se entrevistó a cinco profesionales de la salud. El análisis identificó los siguientes 6 ejes temáticos, en torno a los cuales se organiza este artículo: 1.
